  • It is also indicated for use in combination with insulin in patients whose blood sugar cannot be managed with diet, exercise, and oral hypoglycemic agents. However, combining Glims with insulin increases the risk of hypoglycemia.
✔️ Pharmacology:

Glimepiride, a sulfonylurea antidiabetic agent, lowers blood sugar by stimulating insulin release from functional pancreatic beta cells. It improves beta cells' sensitivity to glucose, enhancing insulin secretion. Additionally, Glimepiride reduces hepatic glucose production, improves peripheral tissue insulin sensitivity, and increases glucose uptake. Its effects can last up to 24 hours in non-fasting diabetic patients.

The dosage of Glimepiride depends on the desired blood sugar level, and the lowest effective dose should be used to maintain metabolic control. Regular blood and urine glucose checks are needed for dose adjustments.

  • Initial dose and titration: Typically, the starting dose is 1 mg daily, with gradual increases based on blood sugar monitoring, adjusting the dose every 1 to 2 weeks: 1 mg -> 2 mg -> 3 mg -> 4 mg -> 6 mg.
